YOUR FIRST OFFICE VISITWhen you schedule your first appointment, we will send you a "New Patient Packet" that contains a first visit guide and several forms on which to detail your medical history. Please complete these forms and bring them with you to your first appointment. This will help us better understand your symptoms and make sure your time with us is spent as efficiently and effectively as possible. You can also click on the following links to access the forms online (requires Adobe Reader). Simply print the forms and fill them out. You should expect your first appointment to last
approximately one hour, during which we will conduct an extensive medical
interview and, typically, perform a pelvic examination. Depending on the nature
of your condition, we also may ask your permission to obtain a urine specimen
with a catheter. Once we have acquired adequate information about your concerns,
we may require specialized testing to make a definitive diagnosis. These tests
will be scheduled at a later date. Click here for descriptions of some of the Specialized
Testing we offer. Insurance
We ask that you arrive 15 minutes before your appointment is scheduled to complete our insurance information sheet. You can save time by printing the form from our website, filling it out, and bringing it with you. If your insurance company requires a referral from your primary physician, please be aware that this is your responsibility.
